Schools are required by Massachusetts General Law to provide vision, hearing, body mass index (BMI), postural, and Brief Intervention and Referral to Treatment (SBIRT) health screenings to all students. Caregivers will be notified if a referral is necessary.

Please complete and submit this form only if you do NOT want your scholar to participate in state mandated health screenings. If your scholar is opting out, you must submit the form by November 1, 2026. If you want your scholar to participate, no action is needed.
